Abstract: 

Background: Ingestion of corrosive chemicals is a major problem, especially in developing countries. Ingestion may be deliberate as suicide attempts (mostly in adolescents and adults) or accidental (mostly in children). Case presentation: A 50-year-old male patient was brought to the APC – he was suffering from vomiting following ingestion of NaOH. On examination: he complained of severe dysphagia, drooling, hoarseness of voice, severe pain, and a burning sensation in his mouth. The vital signs were heart rate of 100 beats /minute, blood pressure 140/90 mmHg, temperature 37°C, respiratory rate of 26/min, and GCS was 15. The patient was stabilized on intravenous treatment and NBO. Follow-ups were carried out for all vital signs, and laboratory tests were carried out, with an initial improvement in his condition. After 6 days, the patient vomited vigorously, and tissues extruded from his mouth, where a complete sloughing of esophageal mucosa occurred. The patient was transferred to the cardiothoracic department where they decided to maintain conservative treatment and NBO for 2 weeks. Later, the patient was prepared for gastrostomy followed by colon transposition. The patient improved and was discharged with a follow-up regimen. Discussion: Esophagitis Dissecans Superficialis (EDS) is a rare esophageal lesion characterized by sloughing of the esophageal mucosa, there is limited information on EDS resulting from corrosive poisoning. Endoscopy was not performed for fear of perforation and CT was carried out instead of endoscopy. Patients usually have an excellent response to treatment, with proton pump inhibitors and steroids. These are the same management lines that followed before surgical intervention. The surgeons perform an esophagectomy and colonic transposition on the patient to avoid stricture in the future. This is considered an acceptable treatment. Conclusion: Clinical toxicologists should be expecting esophageal sloughing as one of the corrosive ingestion complications within a week of ingestion.

Abstract: 

Introduction: Corrosive gastroesophagitis is one of the major problems in surgery and though multidiciplinary approach is used in these patients, prognosis of such patient is poor. In this retrospective study we selected patients with corrosive gastroesophagitis who had referred to Loqman hospital in the last decade and the results of treatment were analyzed. Methods: We analyzed patients with corrosive gastroesophagitis who had referred to Loqman hospital in the last decade retrospectively. Factors included age, sex, grade, substance use, surgery option and results of endoscopy. Results: In this study, 88 patients were evaluated. Most of the patients were men (64%) in the 31-40 years age group and the most common substance involved was acid (42%), cleaning (25%) and alkaline (15%) solutions. Mortality on the base of grading was: grade I= 0%, grade II = 8.5% and grade III= 64%. Endoscopy results in operated group were as follows: 48% ulcer and 26% stricture, while in the non operated group 7% had ulcers and 46% strictures. Mortality was 59% in the operated group and 80% in the non operated group. But all grade III patients had dysphagia. Conclusion: In grade II corrosive esophagitis, gastritis surgery is the best option for decreasing morbidity, but in grade III patients, surgery has a very poor prognosis.

Abstract: 

Background: Acute poisonings with corrosive substances cause serious chemical injuries to upper gastrointestinal tract, the most common site being the oesophagus and the stomach. The degree and extent of damage depends on several factors like the type of substance, the morphologic form of the agent, the quantity, and the intent. In the acute stage, perforation and necrosis may occur. Injury to UGI tract due to ingestion of acidic corrosive substances is common in India. Acute corrosive intoxications constitute a major problem in clinical toxicology since the most commonly affected population are the young with psychic disorders, suicidal intent and alcohol addiction.Case Report: We report a fatal case of accidental corrosive acid ingestion with a brief review of literature.Conclusion: Ingestion of a corrosive substance can produce severe injury to the gastrointestinal tract and can even result in death. Stringent legislation is necessary in developing countries to curtail unrestricted access of adults to dangerous corrosive chemicals.

Abstract: 

Crude oil is an energy resource that mostly is composed of the hydrocarbons and trace corrosive compounds as well has found some cruxes in the petroleum refining industry because of the vast applications of the different types of metals. There is expectation to investigate the effect of two different types of crude oils on the corrosion of seven types of the selected ferrous metals in the pertinent research. The concerned corrosive properties of both crude oils have tested by standard methods and instruments, also the chemical compositions of the selected ferrous metals have tested. The corrosion rates of a series of the similar size prepared metal coupons have tested after limited immersion time periods as three trials in crude oils by the weight loss method since aiding the microscopic analysis to identify the corrosion compounds. Apart from those speculations, the decayed metallic elemental mounts from the metals into crude oils and the reductions of the initial hardness of metal coupons have tested. The obligatory results shows the relatively higher corrosion rates from carbon steels, relatively lower corrosion rates from stainless steels in both crude oils, formation of FeS in most of observations, decay of ferrous and copper into crude oils from some metals, and slight reductions of the initial hardness of metal coupons due to the formation of the corrosion on the metal surfaces.
